Form G-1145 is a one-page request for an email or text message when a USCIS Lockbox facility accepts a paper application or petition. You clip it on top of the first page of your package, and within 24 hours of acceptance USCIS sends one message per form you filed, showing the receipt number and how to check case status. It asks only for your name, email address and mobile number, and carries no fee. It does not grant any status or benefit, and the paper receipt notice (Form I-797C) still arrives by mail, generally within 10 days, as proof that the case is pending. Its edition, 09/26/14, is unchanged for years, which is why its PDF footer shows that date.
How to file Form G-1145
Clip to a paper package
Complete it and clip it on top of the first page of your application. One message is sent for each form in the package.
“Complete the information below and clip this form to the first page of your application package.”
Filing online instead?
A USCIS online account shows receipts and case updates in the account, so G-1145 is for Lockbox paper filings.
“This service is available for applications filed at a USCIS Lockbox facility.”
Why our filler is off for this form: USCIS says its forms are optimized for Adobe Acrobat Reader, and part of this PDF is read-only by design (copied headers, barcodes, signature boxes). We turn our fill on per form only after the output re-opens in Acrobat Reader with every value intact and a matching barcode. Until then, Adobe Reader on a computer is the free tool USCIS points to.
