Form 4506-C lets you authorize a third party, usually a mortgage lender, to get your tax transcripts from the IRS through the Income Verification Express Service (IVES). You name the IVES participant on line 5a and the client company that will receive the transcripts on line 5d, then choose the transcript type and up to four years or periods. The IVES participant, not you, faxes the signed form to the IRS. It is the third-party route that replaced transcript mailings to lenders on Form 4506-T. Fields have character limits (12 letters for a first name, 22 for a last name), and the form must reach the IRS within 120 days of your signature. The current revision is October 2022.
