Form 4506-T asks the IRS for a transcript of your tax information: a return transcript (most line items as filed), an account transcript (payments, penalties and adjustments), a record of account (both combined), a verification of non-filing, or a transcript of W-2, 1099, 1098 or 5498 information. It is free and it is not a copy of the return itself; for a photocopy you use Form 4506. Since July 2019 the IRS mails transcripts requested this way only to your address of record, so a lender or other third party that needs a transcript sent to them usually uses Form 4506-C through the IVES program instead. Individuals can often get transcripts faster with the IRS's online Get Transcript tool.

The IRS stopped mailing transcripts to third parties in July 2019. If a lender needs one, it can use an IVES participant (Form 4506-C).

Form 4506-T (Rev. 4-2025), Signature and dateNot sure? Record of Account.
Transcripts are masked (for example only the last four digits of an SSN show) but full financial figures appear. The instructions suggest the most detailed option if you are unsure.
“If you are unsure of which type of transcript you need, request the Record of Account, as it provides the most detailed information.”
